NO-SHOW & CANCELLATION

THIS NOTICE DESCRIBES OUR “NO-SHOW & CANCELLATION POLICY & PROCEDURE FOR MEDICAL AND COSMETIC VISITS”
PLEASE REVIEW IT CAREFULLY

At The Dermatology House, our goal is to provide quality medical and cosmetic dermatologic services in a timely manner. We schedule our appointments so that each patient receives the right amount of time to be seen by our physicians and staff. That’s why it is very important that you keep your scheduled appointment with us and arrive on time. As a courtesy, and to help patients remember their scheduled appointments, we will send a series of voice messages, text messages and email reminders in advance of the appointment time.

 

We have implemented a no show and cancellation policy which enables us to better utilize available appointments for our patients in need of care. The following policy is with regard to patients who fail to keep their scheduled office visit appointment, for medical and/or cosmetic services.

 

Please be courteous and call our staff promptly if you are unable to attend an appointment. This time will be reallocated to someone who is in urgent need of treatment. Available appointments are in high demand and your early cancellation will give another person the possibility to have access to timely care

 

Patients who fail to show for their scheduled appointment or did not notify the office within 24 hours of their scheduled appointment time, shall be subject to a “No Show/Cancellation” fee of $50.00. In the event of an actual emergency and prior notice could not be given, consideration will be given, and a one-time exception may be granted.

 

These fees are not covered by insurance companies and are therefore the sole responsibility of the patient.

 

To cancel or reschedule appointments call The Dermatology House at 435-602-1918. If you have any problems getting through, you can leave a message with your name, appointment date and cancellation reason or request for rescheduling.

 

This Notice was published and becomes effective on 1/1/2020.
